Addison County Home Health and Hospice is celebrating its 51st year of providing compassionate, kind cutting-edge home care and hospice care to the residents of Addison County in their own homes. In the past year, their clinicians have been invited into 1,478 patients’ homes and they travelled 508,974 miles as they made 57,454 visits to homebound patients. Their goal is keep their patients comfortable at home for as long as they are safe and comfortable. They teach new parents to care for children, help patients who have just come home from the hospital with new medications or medical equipment, help orthopedic patients before and after surgery and support patients and their families at the end-of-life.

Community involvement is a priority for this organization. They sponsor 60 foot care clinics for residents sixty and above in various locations around the county. Their licensed therapy team is available to provide training for companies and organizations in body mechanics to ensure proper lifting and fall prevention. Their social workers and nurses regularly provide assistance in completing advance care documents which brings peace of mind. Their employee engagement committee members volunteer in the community serving meals at the Charter House and at the summer camp lunch program, holding food drives for HOPE, volunteering at the Ripton Run and fundraising for Homeward Bound’s Woofstock event. Additionally, several ACHHH employees volunteer their free time serving on boards of directors for area non-profits.
